General objective of the mission
Strengthening the capacity of midwives and the organization of the maternity ward of Ruhengeri Hospital
Specific objectives
Under the responsibility and with the support of the Clinical Director of the hospital:
1. Support midwives in the organization of admission services and delivery room: reception and referral of patients, supervision of midwives in admission room and delivery room
2. Skills transfer: training of midwives, nurses, students in patient care throughout the course within the maternity hospital from admission to the delivery room in accordance with the Rwandan National Protocol and international protocols
3. Sensitize and train midwives in the practice of cupping, forceps when indicated
4. Ensure emergency consultations in the admission room
5. Evaluate the flow, communication, interaction between the delivery room and the neonatology department and make proposals to improve existing practices
6. Assess the quality of the medical support of the beneficiaries of the One stop center and the flow (in terms of communication and confidentiality) between the IOSC and the gynecology department
7. Assess the quality of gender integration in care within the service and make proposals for its improvement
8. Participate in the definition of the project objectives within the obstetric gynecology department led by Expertise France
Deliverables expected at the end of the mission:
1. The diagnosis will be made within the first 2 weeks of the mission and will be accompanied by a report showing the results of the diagnosis and the training plan :
· Diagnosis on the organization of reception and orientation of patients in the admission room
· Assessment of patient management and supervision in the delivery room
· Assessment of training needs of midwives, nurses in the delivery room
· Assessment of post-partum care
· Assessment of gender integration aspects (how care practices and patient’s pathway integrate gender challenges? Capacity and roles of medical and paramedical staff in prevention of the GBV? Etc.)
· Training report: knowledge transmitted to midwives (management of labor in the delivery room, management of the orientation of patients received in admission)
2. Monthly mission report: Number of vaginal deliveries achieved, number of patients requiring a transfer to the CHU-K; details of acts performed and acts not performed due to lack of means (human and equipment, rate of caesarean section, instrumental extractions and normal vaginal deliveries).
Number of patients seen in the postpartum period with points of improvement in management.
3. Final mission report: recommendations in the management of the delivery room and post-partum for the control of good practices.
Rwanda’s National Strategic Plan for the Health Sector (2018 - 2024) which has been defined by the Ministry of Health aims to improve four key identified health development sectors by 2024:
- Increase social protection throughout life;
- Expand access to essential health services for communicable and non-communicable diseases;
- Strengthening health systems;
- Strengthen health security and resilience of the health system.
In addition to this strategic plan, a human resources training program in the health sector will provide 10-year (2028) training for approximately 6,000 medical and paramedical professionals.
In June 2021, the Ministry of Finance and Economic Planning sent a request to AFD to finance the first phase of a reconstruction project for the Ruhengeri Hospital. The current capacity of the hospital is 350 beds and the objective of the project is to build a modern structure of 550 beds to offer a wide range of activities: medical imaging, outpatient consultations (dentistry, surgery, maternity and neonatology, physiotherapy, pediatrics and ENT), in-house and patient rooms, emergency departments, gynecology and obstetrics, intensive care, infection prevention and control services.
Following the AFD identification mission held in February 2022, the authorities validated the opportunity to support the Rwandan authorities in this project. This support will take the form of a loan dedicated to the rehabilitation of the hospital for a maximum amount of 70 million euros.
In support of the loan to rehabilitate the Ruhengeri hospital, AFD financing has been mobilized to improve access to quality care throughout the country. In this context, funding was granted to Expertise France for the implementation of a project on strengthening health human resources at the national level and particularly at the Ruhengeri hospital.
Project design
The overall objective of the project is to improve access to healthcare for the population of Rwanda and specifically in the Northern Province.
The specific objective of the project is:
OS1: Improving the availability and quality of health human resources in Rwanda and more particularly in the province of Musanze.
To implement this project, two main activities are deployed:
Mobilization of short-term medical and paramedical expertise from French Teaching Hospital
Medical and paramedical trainers from French university hospitals will be mobilized in Rwanda to train and share the best practices of their discipline with their Rwandan colleagues. Based on findings and recommendations from the assessment that will be conducted a training program/ plan will be designed and organized on the following medical specialties: emergency; gynecology-obstetrics; adult and pediatric cardiology; pediatric surgery; neonatal resuscitation; interventional imaging etc.
The interventions will target the Ruhengeri Hospital and at the central level in different tertiary health facilities (CHU K, King Faisal Hospital, Military Hospital and CHU de Butaré).
Mobilization of medium-term medical expertise within the Ruhengeri Hospital
In addition to the mobilization of short-term expertise and in order to meet the urgent needs at the Ruhengeri hospital, specialists will be mobilized in the medium term on the following three disciplines:
- Emergencies
- Gynecology-obstetrics
- Pediatrics Neonatology
- Intensive Care Unit
Expertise France seeks to mobilize a specialist in each discipline to ensure continuity of training and care.
The midwife in collaboration with her team, ensures patient care, she guarantees the proper functioning of the maternity while strengthening the capacity of midwives and other paramedics of the service.
Required profile
· At least 8 years' professional experience including experience in humanitarian context and hospital with limited resources and experience in Teaching Hospital setting
· Knowledge on gender integration in healthcare
· Experience as head maternity in hospital is highly desirable
· Knowledge of project management fundamentals and/ or Public Health is an asset
Skills
· Rigor and organizational skills
· Up to date knowledge of current clinical and professional processes and procedures
· Good command of English and French (good writing, synthesis and analytical skills)
· Ability to train, to guide and lead a team
· Type and duration of the mission: First mission of 2 months and 2 missions of 2 weeks each per year until November 2027 (negotiable)
· Position: based in Ruhengeri in the north province of Rwanda.
· Starting date: As soon as possible
· Remuneration: according to the profile and seniority of the selected candidate and Expertise France salary scale
