Mission description

This mission will focus on the fourth priority lever of the project - enhancing healthcare service planning, especially for maternal and child health, based on a geographic and functional assessment of existing capacities, while initiating strategic reflection on hospital infrastructure needs. Reducing maternal and neonatal mortality is a major priority for the Nigerian government, in a context where indicators remain among the most worrying globally. One solution is to strengthen the provision of emergency obstetric and neonatal care (EmONC), through basic (BEmONC) and comprehensive (CEmONC) centers.
Furthermore, the broader modernization of hospital infrastructure remains a strategic challenge to improve the quality of care, the efficiency of technical platforms, and the resilience of the Nigerian health system. With this perspective, the AFD is considering the implementation of a loan program aimed at strengthening the supply of care in three federated states. This component will support this dynamic through two complementary aspects:
• Targeted technical support for improving the EmONC network (BEmONC/CEmONC centers);
• A streamlined exploratory study (of the pre-feasibility type) to identify the needs for strengthening supply, particularly in hospital infrastructure and medical specialties, in certain previously targeted states.
As a result, the mission of the health service planning short-term expert is to:
1. Conduct an evaluation of BEmONC and CEmONC facilities (availability, functionality, staffing, equipment, service continuity).:
Mapping existing BEmONC and CEmONC structures in targeted states, including an assessment of their actual capacity (human resources, equipment, continuity of services);
Analyzing accessibility geographically according to international coverage standards (time and distance between the population and EmONC structures), integrating local constraints (roads, transport, topography). It is important to note that this analysis will depend heavily on the quality and availability of data, including the accurate identification of places of residence, commonly used modes of transport, and road infrastructure;
Inventory of services actually provided (frequency of EmONC signage functions, availability of care, etc.).
2. Facilitate strategic planning of the EmONC network:
Analyze the gaps between the current supply and the normative needs;
Conduct prospective model accessibility modeling (use of GIS for different development scenarios);
Define adaptation scenarios for the EmONC network (rehabilitation, construction, prioritization);
Propose scenarios and a prioritized intervention plan to optimize coverage and functionality (rehabilitation, upgrading, construction, referral linkages) 
3. Contribute to a light pre-feasibility assessment of broader hospital infrastructure needs in targeted states (strategic, not detailed engineering design):
Conduct a preliminary analysis to identify the most urgent needs for general hospital infrastructure in the targeted states:
Document review and analysis of existing data (national plans, regional studies, hospital equipment databases, data on the health needs of the population of these States);
Identify gaps in healthcare provision and territorial distribution (hospital capacity, availability of technical facilities, availability of specialized human resources, etc.) 
Conduct targeted interviews with stakeholders (federal and state authorities, hospital management);
Develop initial hypotheses for prioritizing the strengthening of the territorial offer (geographic areas, types of structures) with a view to possible subsequent intervention within the framework of the loan program.

The approach will prioritize the use of available secondary data (reports, national databases, technical documentation) and consultations with stakeholders (ministries, technical agencies, and state authorities) to refine analyzes without resorting to heavy field data collection. This mission will rely on available data (Federal Ministry of Health, partner states, technical agencies), with a pragmatic and realistic approach adapted to available resources. The expert will also be expected to support the project team in facilitating workshops to develop and review outputs with local stakeholders. This component will be implemented in close collaboration with the FMoH&SW, NPHCDA, NHIA, and other relevant stakeholders. 

This list is not exhaustive, and the tasks and activities mentioned are not restrictive and will be further defined during the mission.

Project or context description

EF in Nigeria
Expertise France (EF) operates in Nigeria within a broader regional framework, notably through its collaboration with ECOWAS. Its activities primarily involve regional or multi-country initiatives funded by the European Union and the AFD Group.
Currently, Expertise France is engaged in the implementation and development of several regional projects, including:
• EPSAO II – Support to the ECOWAS Peace and Security Architecture
• SECFIN Africa – Strengthening AML/CFT systems to combat illicit financial flows in Sub-Saharan Africa
• WASOP – West Africa Sustainable Ocean Program, promoting an innovative and sustainable blue economy
• PEERS – Africa-Europe platform for dialogue on education reforms
• TEI SRHR – Team Europe Initiative on Sexual and Reproductive Health and Rights
• ProTECT – Preventing Trafficking through Enforcement, Cooperation and Training
• MEDISAFE2 - Mitigating the harms and threats posed by Substandard and Falsified Medical Products to Public Health and Public Security in West and Eastern Africa

In addition, the Health Department of Expertise France has launched the implementation of two national projects in Nigeria:
• QUALIMEDS (Quality Uplift for Advancing Local Industry in MEDicine Standards)
• FEXTE Nigeria (in collaboration with the human capital and social development (CHDS) department)
Additional national or regional projects may be launched in the coming years.

FEXTE (Fund for technical expertise and exchange of experiences)
In response to the significant public health challenges, the Nigerian government launched an ambitious health sector reform in late 2023, centered around a Sector-Wide Approach (SWAp). This reform aims to strengthen primary healthcare, particularly maternal and child health, expand universal health coverage, and redesign primary care financing through mechanisms such as the Basic Healthcare Provision Fund (BHCPF). Major international partners, including the World Bank and the Gates Foundation, are supporting this effort.
As part of this dynamic, Nigeria has expressed interest in diversifying its technical partnerships. France has been identified as a complementary partner, especially in under-addressed strategic areas such as health workforce development, pharmaceutical systems, universal health coverage, and maternal and child health.
The overall objective of FEXTE is to contribute to improving equitable access to quality, affordable, and population-responsive healthcare by supporting the reform of the Nigerian health system. The project will be implemented in close coordination with the Federal Ministry of Health and Social Welfare, the NHIA, and NAFDAC, under the guidance of the French Embassy and AFD Nigeria.
More specifically, the project aims to strengthen the capacities of Nigerian institutions to effectively lead, finance, and regulate their health system through four priority levers:
1. Strengthening human resources for health (HRH), with a particular focus on the training of midwives;
2. Structuring a sustainable, equitable, and financially viable universal health coverage (UHC) system;
3. Developing a national health technology assessment (HTA) framework to inform reimbursement decisions, pricing, and pharmaceutical regulation in support of UHC;
4. Enhancing healthcare service planning, especially for maternal and child health, based on a geographic and functional assessment of existing capacities, while initiating strategic reflection on hospital infrastructure needs.

Its ultimate objective is to contribute to equitable access to quality, affordable, and context-appropriate healthcare in Nigeria. 

Required profile

• University Degree in Healthcare, Public Health, Medicine, or other relevant diplomas
• At least 10 years' experience in the field of public health, maternal and child health, health service planning, health systems strengthening and other related fields
• Deep expertise in facility network assessments, particularly in designing and evaluating services for reducing maternal and neonatal mortality, and specialized knowledge of EmONC standards, including BEmONC and CEmONC
• Proven ability to conduct comprehensive evaluations of healthcare supply, service readiness analysis, analyze capacity (human resources, equipment), identify gaps between current provision and normative needs, and develop long-term strategic plans for healthcare network evolution
• Mandatory skill in utilizing Geographic Information Systems (GIS) and data analysis for geographical accessibility studies, prospective accessibility modeling, and analyzing territorial distribution of healthcare provision
• Experience in high-level strategic thinking or pre-feasibility analysis related to hospital infrastructure, medical specialties, and the efficiency of technical platforms
• Proven experience of engagement in international cooperation projects with institutional partners, preferably in Sub-Saharan Africa
• Experience engaging with national authorities, international agencies, and donors
• Computer proficiency in MS Office, in particular MS Word, MS Excel, MS PowerPoint, MS Outlook, and messaging and video conferencing tools
• Fluency in English (both oral and written) required
• Excellent interpersonal and organizational skills
• Proactive, autonomous, and adaptable to changing contexts
• Analytical thinking and capacity for synthesis.

The following experience and skills will be considered as an asset
• Understanding of the Nigerian health context, specifically the role of the Federal Ministry of Health, federated states, and the importance of alignment with broader programs (like the AFD loan program and other relevant reforms).
• Advanced degree in public health with specializations in health service planning
• Experience of participation in similar international projects, particularly in Africa 

Additional information

Working method
•    A total of 40 days of work is expected for this mission, phases of which will be formalised using the findings of the initial diagnostic study, which should last no longer than 15 working days
•    The working language of the project is English 
•    Besides face-to-face consultations with the beneficiaries during the missions in Nigeria, some work will be done remotely, both on the basis of the available documentation and by means of video conferences
•    In Abuja, the expert will be able to work in the project’s premises and may be accompanied to their meetings with the beneficiaries either by the project coordinator and/or national experts
•    The short-term expert may be solicited to participate in meetings with the French Development Agency (AFD) to discuss the project’s intervention strategy and progress 
•    The short-term expert may work in collaboration with the project manager at EF HQ
